Parent Consent form T1
This form is required by your cadet when ever they attend an event or course outside of the normal cadet parade evenings.
Under normal circumstances your cadet will be issued with on that has been printed with their details. However if it is lost or you require one then this link below will enable you to print a blank on.
Please ensure the telephone number beside your signature is valid during the period of the event, in case we need to contact you.

This form is used to accompany the T1 consent form should your cadet be taking an medication during the event. Unfortunately without this filled in your cadet will not be able to stay on course for health reasons.
